What do I need to bring with me to my first appointment?List Item 1
If your insurance is covering the cost of your treatment, please be sure to bring your insurance card. If you are covered by Workers’ Compensation, please bring your claim number and the contact information for your case manager. If you are covered by auto insurance or an attorney lien, please bring this information with you.

What should I wear?List Item 2
Patients are advised to wear loose fitting clothing so they are easily able to expose the area that will be evaluated and treated. Please dress comfortable, keeping in mind that you will be moving your body and performing exercises.
If you are scheduled for a session in the pool, please bring a swimsuit or a tee shirt and shorts that you want to wear in the pool (either is just fine). Your hair may get wet, if you do not wish for this to happen, please bring a swim cap.

What can I expect during my first visit?List Item 3
When you first arrive at your appointment, you will be asked to complete paperwork. You can download these forms from our website if you wish to complete them prior to your appointment. Please visit the link below. If you elect to fill out these forms day on the day of your evaluation, please plan to arrive fifteen minutes prior to the scheduled session to allow adequate time for completion.
Second, we will review your referral for Physical Therapy and make a copy your insurance card. You will then be seen by a Champion Rehab Therapist for your initial evaluation.
The Therapist will discuss the following:
- Medical history
- Current medication review
- Problems/complaints
- Details on pain intensity: is there anything that increases/decreases the problem?
- Your therapy goals
Your Therapist will then perform an evaluation, which may include some of all of the following:
- Palpation (Touching around the area of pain/problem)
- Range of Motion (Movement of joints to check for any restrictions)
- Muscle Testing (Checking for muscle pain and weakness)
- Neurological Screening (Review of how the nerves are communicating with the muscles. This may include reflex testing)
- Posture Assessment
Your Therapist will create an individualized plan for you based on this assessment. This plan includes both the patient and doctor’s input. Your personalized therapy plan may include how many times a week you should see a Therapist, how long your rehabilitation is anticipated to be, home programs and what is expected upon discharge from therapy.

How long will each treatment last?List Item 4
Treatment sessions will vary but typically last between 30 and 60 minutes each.

How many visits will I need?
The number of visits needed per patient varies greatly. Some patients only require one visit, while others may need months of care. You will be evaluated during your first appointment and this will be further discussed between you and your Therapist. If you require on-going treatment, you will be re-evaluated frequently and provided a progress report with our recommendations.

Who will I be seeing during my appointment?
You will be evaluated by one of our licensed, compassionate and highly trained Therapists. Unlike other clinics, where you may see someone new every visit, you will be treated by the same Therapist throughout your treatment with Champion Rehab. If you need continued treatment, our Therapists are also able to provide in-home therapies. Patients appreciate this practice as it allows them the opportunity to build a trusting, comfortable relationship with the individual they are working with.

What happens if my pain/problem returns?
First of all, know that this is not uncommon. If you have a flare up, please give us a call. We will be able to suggest an appropriate plan to help. This may include coming in to see us, visiting your doctor, or a simple modification to your daily activities.

Can I go to any clinic for my Physical, Occupational, or Speech Therapy?
You have the right to choose where you receive Therapy. We advise all patients to make sure their payment source (insurance) is accepted at their intended Rehabilitation Clinic. If you are seeking a specific kind of treatment/specialty, we suggest you do research to find out what clinics in your area provide.
The best thing to do is call us and we can answer your questions directly.

How does the billing process work?
After you are seen for treatment, the following occurs:
- Your Therapist bills your insurance company, Workers’ Comp, or charges you based on services received. Your charges are based on Common Procedure Terminology (CPT) codes.
- These codes are transferred to a billing form that is sent to the payer source.
- The payer makes a payment according to an agreed upon fee schedule.
- An Explanation of Benefit (EOB) is sent to the patient and physical therapy clinic with the payment and any remaining balance due by the patient.
Please note that we are here to assist and answer any questions you may have during the billing/payment process.

What will I have to do after my treatment?
Some patients will be asked to continue with home exercises, while others may choose to continue therapy with a gym exercise program. Some patients will complete their rehabilitation and return to normal activities immediately.
We suggest that you discuss your goals with your Therapist during your initial treatment so he/she can develop the right program for you.

Where should I park?
Our address is 520 First Street NE, Sartell MN 56377. Please enter at the MAIN ENTRANCE sign and proceed straight to the entrance marked “Rapid Recovery/Champion Rehab” park in front of the building.

Do I need a referral to receive Rehab?
MN is a “direct access” state. This means if you plan to pay privately, or with an insurance that is not medicare, you can make an appointment with a Physical Therapist without a referral from your doctor. Please note that Occupational and Speech Therapy do require a referral.
